Updated "intake forms" for easier case submission

What's new?

"Intake forms" will now be referred to as "Treatment details" and require fewer questions (from 13 down to 6) to be answered in order for you to submit a case.

Additionally, we have also reduced the number of questions for Additional Aligner cases down to just 4.

Why did we make this update?

We want to continue simplifying the case submission process, as well as the process for requesting Additional Aligners, to be easy for our doctors to complete.

When is this change happening?

These changes went live November 9th, 2022.
